For our final Project Impact meeting, we will discuss plan evaluation based on completed action steps and highlight anecdotal wins that demonstrate positive change in practice or outcomes. We will discuss and problem-solve for any challenges.  During the Check phase, teams will evaluate progress through data reviews, progress-monitoring tools, feedback cycles, and scheduled reflection meetings, and make any needed tweaks to the implementation plan.
